We have an exciting opportunity for a Habilitation Officer for Visually Impaired individuals to join our team to help promote independence through reablement programmes that enable young individuals and their families to overcome the effects of visual loss.
Working 18.5 hours per week flexibly, including additional hours from time to time to meet the needs of the job, and some unsocial hours working you will have the opportunity to conduct visits to Children/Young People and their families/carers, liaising with other professionals as well as experience in multi-agency working.
The post will respond to referrals received and will be responsible for carrying and maintaining a manageable caseload. This will consist of initial contact visits following receipt of the Certificate of Visual Impairment (CVI) as well as referrals from Special Needs Teaching Services, Social Services or other agencies where a more intensive package of rehabilitation is required.
Your skills will enable you to carry out reablement assessments in the SU’s home and use these to design and implement comprehensive reablement programmes for visually impaired adults and young people. You will also have experience of working with individuals with additional disabilities, either on an individual or group basis, in their own homes, school or in the community.
Your experience will mean you are able to teach a wide range of skills including, communication skills, braille, IT and audio equipment as well as range of indoor and outdoor mobility and independence skills.
You will support people with a sight loss with sighted guide techniques, pre cane skills, long cane skills, orientation, independent use of public transport, the use of navigation technology and other mobility aids as well as assessing the need for equipment that will enable people with a sight loss to retain skills and independence.
You will be experienced in developing a comprehensive range of Short Term and Long Term focused reablement programmes for people with a sight loss which have clear timescales and outcomes whilst maximising opportunities to enhance social skills, self-confidence and independence.
Meeting tight deadlines is important to those we support whilst maintaining comprehensive written case records you will be able to work on your own initiative ensuring we meet the requirements of our Service Level Agreements.
We are looking for a self-motivated individual with excellent communication and organisational skills who has the ability to time manage, prioritise caseload and be able to work alone.
